RIVERTON, KANSAS / ACCESS Newswire / July 7, 2025 / Spring River Mental Health & Wellness (SRMHW) is committed to protecting the privacy and security of its employees' and clients' personal and protected health information. We are writing regarding a data privacy incident that may have affected the personal and protected health information of certain SRMHW clients and current and former employees.
What Happened
On November 26, 2024, SRMHW discovered suspicious activity in its network. Upon discovery, SRMHW took immediate action to secure its systems and partnered with cyber incident response professionals to investigate the incident. As part of the investigation, SRMHW learned that certain data may have been accessed or acquired by an unauthorized actor between October 16, 2024, and November 26, 2024. As a result, SRMHW underwent a comprehensive data review to determine what information was involved and to whom that information belonged. On May 29, 2025, SRMHW determined that personal and protected health information of certain SRMHW clients and current and former employees was included in the data set.
What Information Was Involved
The information involved varied by individual but may have included individuals' names, Social Security numbers, dates of birth, driver's license or state ID numbers, individual taxpayer identification numbers, financial account information, payment card information, patient account numbers, Medicaid and Medicare numbers, treatment or medical diagnosis information, provider name, prescription information, medical record numbers, and health insurance member/policy numbers.
Clients whose information was involved were notified on July 7, 2025.
What is SRMHW Doing?
SRMHW takes this incident seriously. While SRMHW had security policies and protections already in place on its network, SRMHW is reviewing and strengthening those security postures. SRMHW has implemented additional measures to safeguard against evolving threats moving forward. SRMHW has reported the incident to federal law enforcement, the Kansas OITS, and the Department of Health and Human Services, Office for Civil Rights. Additionally, SRMHW is offering complimentary credit monitoring and identity protection services, free of charge, to individuals whose Social Security numbers were involved through IDX. Should individuals have any questions or concerns, they may contact SRMHW's dedicated assistance line with IDX at 1-800-939-4170.
What Impacted Individuals Can Do
As a general matter, one should remain vigilant by reviewing credit reports, financial account statements, and explanation of benefits forms for suspicious activity and to detect errors. We also remind everyone that individuals are entitled to one free credit report annually from the each of the three major credit reporting bureaus, TransUnion, Experian, and Equifax. To order a free credit report, visit www.annualcreditreport.com or call 1-877-322-8228.
Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and the steps to take to protect personal information by contacting the credit reporting bureaus, the Federal Trade Commission (FTC), or state Attorneys General. The FTC also encourages those who discover that their information has been misused to file a complaint with them.
